Rofuclav (Cefuroxime + Clavulanic Acid): A Broad-Spectrum Antibiotic with Beta-Lactamase Inhibitor
Rofuclav is a combination antibiotic containing Cefuroxime (a second-generation cephalosporin) and Clavulanic Acid (a beta-lactamase inhibitor). This combination is effective against a wide range of Gram-positive and Gram-negative bacteria, including many strains that produce beta-lactamase enzymes, which would otherwise inactivate Cefuroxime alone. It is indicated for the treatment of various bacterial infections.
How Rofuclav Works (Pharmacology)
Rofuclav's effectiveness stems from the synergistic action of its two components:
- Cefuroxime:
- It is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by interfering with the transpeptidation process, leading to the death of the bacterial cell.
- It is active against a broad spectrum of susceptible organisms.
- Clavulanic Acid:
- A naturally derived beta-lactamase inhibitor.
- It binds irreversibly to and inactivates beta-lactamase enzymes produced by some bacteria. These enzymes would normally break down Cefuroxime.
- By protecting Cefuroxime from degradation, Clavulanic Acid extends the spectrum of Rofuclav to include infections caused by beta-lactam-resistant bacteria.
Key Indications & Benefits
Rofuclav is indicated for the treatment of infections caused by sensitive bacteria, including:
- Upper Respiratory Tract Infections: Pharyngitis/Tonsillitis, Acute Bacterial Otitis Media (including beta-lactamase producing strains), Acute bacterial maxillary sinusitis (non beta-lactamase producing strains).
- Lower Respiratory Tract Infections: Pneumonia (caused by various susceptible organisms, including beta-lactamase producing Haemophilus influenzae), Acute bacterial exacerbation of chronic bronchitis, and secondary bacterial infections of acute bronchitis.
- Skin and Skin-Structure Infections: Uncomplicated infections caused by Staphylococcus aureus (including beta-lactamase producing strains) or Streptococcus pyogenes.
- Urinary Tract Infections: Uncomplicated UTIs caused by E. coli or Klebsiella pneumoniae.
- Bone and Joint Infections: Caused by Staphylococcus aureus.
- Uncomplicated Gonorrhoea: Caused by penicillinase-producing and non-penicillinase producing strains of Neisseria gonorrhoeae.
- Early Lyme Disease (erythema migrans): Caused by Borrelia burgdorferi.
- Septicemia: Caused by Staphylococcus aureus, Streptococcus pneumoniae, E. coli, Haemophilus influenzae, and Klebsiella spp.
- Meningitis: Caused by Streptococcus pneumoniae, Haemophilus influenzae, Neisseria meningitidis, and Staphylococcus aureus.
- Switch therapy: From injectable to oral antibiotic treatment.
Dosage & Administration
Rofuclav tablets can be taken without regard to food. Dosage varies by age, type, and severity of infection. Always consult a registered physician for specific dosage instructions.
- Adolescents and adults (13 years and older):
- Pharyngitis/tonsillitis, Acute bacterial maxillary sinusitis, Uncomplicated urinary tract infections, Community acquired pneumonia: 250 mg twice daily.
- Acute bacterial exacerbation of chronic bronchitis, Secondary bacterial infections of acute bronchitis, Uncomplicated skin and skin structure infections: 250-500 mg twice daily.
- Uncomplicated Gonorrhoea: 1000 mg single dose.
- MDR Typhoid Fever: 500 mg twice daily for 10-14 days.
- Early Lyme disease: 500 mg twice daily for 20 days.
- Paediatric Patients (3 months to 12 years): Dosages are typically weight-based and given twice daily.
- Pharyngitis/Tonsillitis: 20 mg/kg/day twice daily.
- Acute otitis media, Acute bacterial maxillary sinusitis, Impetigo: 30 mg/kg/day twice daily.

Contraindications:
- Patients with known allergy to cephalosporin antibiotics.
- Patients with Pseudomembranous Colitis.
Side Effects:
Rofuclav is generally well tolerated. Most side effects are mild and self-limiting.
- Common: Nausea, vomiting, diarrhea, abdominal discomfort or pain.
- Overgrowth of Non-susceptible Microorganisms: As with other broad-spectrum antibiotics, prolonged administration may lead to overgrowth of resistant bacteria or fungi.
- Rare (<0.2%): Renal dysfunction, anaphylaxis, angioedema, pruritis, rash, and serum sickness-like urticaria.
Pregnancy & Lactation:
- Pregnancy: While all antibiotics should ideally be avoided in the first trimester, Rofuclav can be safely used in later pregnancy to treat urinary and other infections if clearly needed.
- Lactation: Cefuroxime-Clavulanic Acid is excreted into breast milk in small quantities. The possibility of sensitizing the infant should be considered.
Precautions & Warnings:
- Diuretics: Cefuroxime should be given with care to patients receiving concurrent treatment with potent diuretics.
- History of Colitis: Use with caution in patients with a history of colitis.
- Hypersensitivity: Administer with care to patients with a history of allergy to penicillin, as there is a possibility of cross-sensitivity between cephalosporins and penicillins.
Drug Interactions:
- Probenecid: Concomitant administration increases the area under the serum concentration-time curve (AUC) of Cefuroxime by 50%.
- Drugs that reduce gastric acidity (e.g., antacids, H2 blockers, proton pump inhibitors): May result in lower bioavailability of Cefuroxime and can counteract the beneficial effect of postprandial (after-meal) absorption.
