Rofixim (Cefixime): A Third-Generation Cephalosporin Antibiotic

Rofixim contains Cefixime, a third-generation semisynthetic cephalosporin antibiotic for oral administration. It exhibits strong bactericidal activity against a wide range of Gram-positive and Gram-negative organisms, primarily by interfering with bacterial cell wall synthesis. Rofixim is highly stable in the presence of beta-lactamase enzymes, making it effective against many bacteria resistant to penicillins and some cephalosporins.


How Rofixim Works (Pharmacology)

Cefixime kills bacteria by:

  • Inhibiting Cell Wall Synthesis: It interferes with the synthesis of the bacterial cell wall, a crucial component for bacterial survival, leading to cell lysis and death.
  • Beta-Lactamase Stability: Cefixime is highly stable against beta-lactamase enzymes produced by some bacteria. These enzymes typically break down many penicillin and some cephalosporin antibiotics, rendering them ineffective. Rofixim's stability means it remains active against such resistant strains.
  • Absorption: Approximately 40% to 50% of the drug is absorbed after oral administration, whether taken with or without food.

Key Indications & Benefits

Rofixim is indicated for the treatment of acute infections caused by susceptible microorganisms, including:

  • Upper Respiratory Tract Infections (URTI): Such as otitis media (middle ear infection) and other URTIs where the causative organism is known or suspected to be resistant to common antibiotics, or where treatment failure carries significant risk.
  • Lower Respiratory Tract Infections (LRTI): Such as bronchitis.
  • Urinary Tract Infections (UTI): Including cystitis, cystourethritis, and pyelonephritis (kidney infection).

Clinical efficacy has been demonstrated against common pathogens including Streptococcus pneumoniae, Streptococcus pyogenes, Escherichia coli, Proteus mirabilis, Klebsiella species, Haemophilus influenzae (beta-lactamase positive and negative), Moraxella catarrhalis (beta-lactamase positive and negative), and Enterobacter species.


Dosage & Administration

The absorption of Cefixime is not significantly affected by food. The usual course of treatment is 7 days, which can be extended up to 14 days if needed. Always consult a registered physician for specific dosage instructions.

  • Adults and children over 10 years: Recommended dosage is 200-400 mg daily, depending on infection severity, given as a single dose or in two divided doses.
  • Elderly: Same dose as adults, but renal function should be assessed, and dosage adjusted in severe renal impairment.
  • Children (general recommendation):
    • Recommended dosage: 8 mg/kg/day, administered as a single dose or in two divided doses.
    • Suspension guide:
      • 6 months up to 1 year: 3.75 ml daily
      • 1-4 years: 5 ml daily
      • 5-10 years: 10 ml daily
    • Typhoid: 5 mg/kg body weight twice daily for 10-14 days.
    • Children weighing > 50 kg or > 10 years: Treat with adult dose (200-400 mg daily).
    • Children aged less than 6 months: Safety and efficacy have not been established.
  • Dosage in Renal Impairment:
    • Creatinine clearance â‰Ĩ 20 ml/min: Normal dose and schedule.
    • Creatinine clearance < 20 ml/min: Do not exceed 200 mg once daily.
    • Patients on CAPD or hemodialysis: Same recommendation as for creatinine clearance < 20 ml/min.

Important Considerations & Warnings

It is crucial to discuss your full medical history with your doctor before taking Rofixim.

Contraindications:

  • Patients with known hypersensitivity to cephalosporin antibiotics.

Side Effects:

Rofixim is generally well tolerated, with most adverse reactions being mild and self-limiting.

  • Gastrointestinal disturbances (most frequent): Diarrhea and stool changes (more common with higher doses). Less frequently: nausea, abdominal pain, dyspepsia, vomiting, flatulence. Pseudomembranous colitis has been reported (a more severe form of diarrhea).
  • Central Nervous System: Headache and dizziness.
  • Hypersensitivity reactions: Rash, pruritus (itching), urticaria (hives), drug fever, and arthralgia (joint pain). These usually subside upon discontinuation of therapy.
  • Hematological and clinical chemistry: Infrequent and reversible reports of thrombocytopenia (low platelets), leukopenia (low white blood cells), and eosinophilia. Mild transient changes in liver and renal function tests have been observed.
  • Miscellaneous: Genital pruritus and vaginitis.

Pregnancy & Lactation:

  • Pregnancy: There are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed, as animal reproduction studies are not always predictive of human response.
  • Lactation: It is not known if Cefixime is excreted in human milk. Caution should be exercised when administering to a nursing mother.

Precautions & Warnings:

  • Hypersensitivity: Administer with caution to patients who have shown hypersensitivity to other drugs.
  • Penicillin Sensitivity: Cephalosporins should be given with caution to penicillin-sensitive patients due to partial cross-allergenicity between penicillins and cephalosporins. Severe reactions (including anaphylaxis) have occurred with both classes.
  • Allergic Reaction: If an allergic effect occurs with Rofixim, discontinue the drug and treat the patient with appropriate agents if necessary.
  • Renal Impairment: Administer with caution in patients with markedly impaired renal function.
  • Antibiotic-Associated Diarrhea/Colitis: Treatment with broad-spectrum antibiotics can alter normal colon flora, potentially leading to overgrowth of Clostridium difficile, which produces a toxin that causes antibiotic-associated diarrhea.

Drug Interactions:

  • Anticoagulants: Increases in prothrombin times have been noted in a few patients receiving Rofixim concurrently with anticoagulants. Care should be taken in such patients due to increased bleeding risk.

Overdose Effects

  • No specific antidote exists.
  • Management: Gastric lavage may be indicated. Rofixim is not significantly removed from circulation by hemodialysis or peritoneal dialysis.
  • Adverse reactions in healthy volunteers receiving single doses up to 2 g did not differ from those seen at recommended doses.
