Indications

Recodep is indicated for the treatment of patients with moderate to severe depression that is associated with moderate to severe anxiety. Symptoms that are likely to respond to the medication within the first week include insomnia, feelings of guilt, agitation, anxiety, suicidal thoughts, and anorexia.


 

Pharmacology

Recodep is a combination of two drugs:

  • Amitriptyline: A tricyclic antidepressant that works by inhibiting the reuptake of norepinephrine and serotonin in the brain, thereby increasing their levels and producing an antidepressant effect.
  • Chlordiazepoxide: An anxiolytic (anti-anxiety) medication that enhances GABA-mediated chloride influx through GABA receptor channels. This causes membrane hyperpolarization, which leads to sedative and anxiolytic effects.

 

Dosage & Administration

The optimal dosage varies based on the severity of symptoms and individual patient response. Once a satisfactory response is achieved, the dosage should be reduced to the minimum amount needed to maintain remission. The larger portion of the total daily dose can be taken at bedtime, and in some patients, a single bedtime dose may be sufficient. An initial dose of 3 or 4 tablets daily in divided doses is considered satisfactory.


 

Interaction

The amitriptyline component of Recodep may block the antihypertensive action of guanethidine or similar compounds.


 

Contraindications

Recodep is contraindicated in patients with hypersensitivity to either benzodiazepines or tricyclic antidepressants. It should not be used at the same time as a monoamine oxidase inhibitor (MAOI).


 

Side Effects

Common symptoms of depression, such as anorexia, fatigue, and lethargy, have been reported as side effects.

  • Most frequent adverse reactions: Drowsiness, dry mouth, constipation, blurred vision, dizziness, and bloating.
  • Less common adverse reactions: Vivid dreams, impotence, tremor, confusion, and nasal congestion.

 

Pregnancy & Lactation

The safe use of Recodep during pregnancy and lactation has not been established.


 

Precautions & Warnings

  • Seizures: Use with caution in patients with a history of seizures.
  • Thyroid: Close supervision is required for hyperthyroid patients or those taking thyroid medication.
  • Renal/Hepatic Impairment: The usual precautions for treating patients with impaired kidney or liver function should be observed.
  • Suicidality: All pediatric patients on antidepressants should be closely monitored for clinical worsening, suicidality, and unusual changes in behavior.
  • Withdrawal: Abrupt discontinuation of chlordiazepoxide can lead to withdrawal symptoms similar to those of barbiturates and alcohol.

 

Use in Special Populations

  • Pediatric Use: Safety and effectiveness have not been established. The potential risks must be balanced against the clinical need.
  • Geriatric Use: In elderly and debilitated patients, the dosage should be limited to the smallest effective amount to prevent the development of side effects like ataxia, over-sedation, confusion, or anticholinergic effects.

 

Overdose Effects

Overdose can be fatal.

  • Amitriptyline overdose: Critical signs include cardiac dysrhythmias, severe hypotension, convulsions, and CNS depression.
  • Benzodiazepine overdose: Symptoms include sleepiness, confusion, coma, and diminished reflexes. Withdrawal symptoms can occur upon abrupt discontinuation of either chlordiazepoxide or amitriptyline.

 

Therapeutic Class

Recodep is a combined anxiolytic and antidepressant drug.


 

Storage Conditions

Store in a cool and dry place, protected from light. Keep all medicines out of the reach of children.
