Omezole (Omeprazole): Advanced Proton Pump Inhibitor (PPI)

Omezole contains Omeprazole, a specialized proton pump inhibitor (PPI). It targets the final step of gastric acid production by blocking the $H^+/K^+$ ATPase enzyme system (the "proton pump") inside the stomach's parietal cells.

Following an oral dose, it begins suppressing acid production within one hour, reaches peak efficacy at two hours, and provides continuous relief for up to 72 hours. After stopping the medication, normal stomach acid levels return gradually over 3 to 5 days.

📋 Indications: Targeted Acid Suppression

Omezole is indicated for a wide array of acid-induced gastrointestinal conditions:

  • Active gastric (stomach) and duodenal (intestinal) ulcers.

  • Ulcers caused by taking Non-Steroidal Anti-Inflammatory Drugs (NSAIDs), as well as ongoing prevention for patients with a history of NSAID ulcers.

  • Gastro-Esophageal Reflux Disease (GERD) and severe ulcerating acid reflux (esophagitis).

  • Acid-related indigestion (dyspepsia).

  • Helicobacter pylori bacteria eradication (peptic ulcer disease).

  • Zollinger-Ellison Syndrome (a rare condition causing extreme overproduction of gastric acid).

  • Prevention of hazardous acid aspiration into the lungs during general anesthesia before surgery.

🥄 Dosage & Administration Regimens

1. Oral Capsules (Standard Administration)

  • Duodenal Ulcers: 20 mg once daily for 4 weeks.

  • Gastric Ulcers: 20 mg once daily for 8 weeks. Severe or recurring cases across both ulcer types may be increased to 40 mg once daily under a doctor's direction.

  • GERD / Reflux Management: 20 mg once daily for 4 to 8 weeks. Long-term maintenance runs at 10 mg to 20 mg daily.

  • Acid Dyspepsia: 10 mg to 20 mg once daily for 2 to 4 weeks.

  • Zollinger-Ellison Syndrome: Starts at 60 mg once daily. Doses exceeding 80 mg daily must be split into two divided doses.

  • H. pylori Triple Therapy Eradication (1 Week Duration): Omezole 20 mg twice daily, combined with one of these physician-prescribed antibiotic pairings:

    • Amoxicillin 1 g + Clarithromycin 500 mg (both twice daily).

    • Clarithromycin 250 mg + Metronidazole 400 mg (both twice daily).

    • Amoxicillin 500 mg + Metronidazole 400 mg (both three times daily).

  • Pediatric Severe Reflux (>1 Year Old): Based strictly on weight. 10 kg to 20 kg uses 10 mg to 20 mg once daily; over 20 kg uses 20 mg to 40 mg once daily for 4 to 12 weeks.

2. Intravenous (IV) Injection & Infusion

Used exclusively when oral medication is not possible or appropriate.

  • Slow IV Injection: Add 10 ml of the provided Water for Injection into the vial. Inject slowly over 2 to 5 minutes (maximum speed of 4 ml/minute). Must be used within 4 hours of mixing.

  • IV Infusion: Dissolve the vial contents into 100 ml of normal saline (stable for 12 hours) or 100 ml of 5% Dextrose (stable for 6 hours). Infuse over 20 to 30 minutes. Never co-administer other drugs in the same IV line.

🛑 Critical Warnings & Diagnostic Exclusion

⚠️ Exclude Malignancy First: If a gastric ulcer is suspected, a medical professional must entirely rule out stomach cancer (malignancy) before starting Omezole. Because this medicine reduces pain and heals tissues rapidly, it can easily mask the symptoms of cancer and dangerously delay a proper diagnosis.

  • Clopidogrel Interaction: Avoid using Omezole if you take the blood thinner Clopidogrel. Omezole blocks the liver enzymes needed to activate Clopidogrel, making your heart medication significantly less effective.

  • Bone Fracture Risks: Long-term, high-dose PPI therapy can increase the overall risk of osteoporosis-related fractures of the hip, wrist, or spine.

⚠️ Side Effects & Drug Interactions

  • Common Side Effects: Generally mild and reversible, including headaches, diarrhea, constipation, nausea, flatulence, or mild skin rashes.

  • Liver-Metabolized Interactions: Omezole can delay how quickly your liver clears Diazepam, Phenytoin, and Warfarin. If you are taking Warfarin or Phenytoin, your doctor may need to monitor your blood levels and lower your doses.

  • pH-Dependent Interactions: By lowering stomach acid, it reduces the absorption of Ketoconazole and increases the absorption of Digoxin by 10%.

🤰 Pregnancy & Lactation

  • Pregnancy: Data from extensive epidemiological studies show no adverse effects on the fetus. Omezole can be safely used during pregnancy if required.

  • Lactation: It is unknown if Omeprazole passes into breast milk. If a doctor determines that Omezole is essential for the mother, breastfeeding should be discontinued.

📦 Storage Conditions

  • Store in a dry place, safely away from direct heat and light. Keep entirely out of the reach of children.
