Nexum MUPS 20 is a tablet containing 20 mg of Esomeprazole (as Esomeprazole Magnesium Trihydrate). Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. Nexum MUPS is manufactured by Square Pharmaceuticals PLC in Bangladesh. The term MUPS stands for Multiple-Unit Pellet System, which refers to the formulation of the tablet containing enteric-coated pellets of esomeprazole.

Indications:

Nexum MUPS is indicated in adults and adolescents (12 years and older) for:

• Gastroesophageal Reflux Disease (GERD):
  • Treatment of erosive esophagitis.
  • Maintenance of healed esophagitis to prevent recurrence.
  • Symptomatic treatment of GERD.
• Risk reduction of NSAID-associated gastric ulcers.
• H. pylori eradication in combination with appropriate antibiotics in patients with duodenal ulcer disease.
• Zollinger-Ellison syndrome and idiopathic hypersecretion.
• Treatment of duodenal and gastric ulcers caused by acid dyspepsia.

Dosage and Administration:

The dosage depends on the condition being treated and the patient's age. Here are some general guidelines for adults (18 years and older):

• Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks. An additional 4-8 weeks of treatment may be considered if healing is not complete.
• Maintenance of Healing of Erosive Esophagitis: 20 mg once daily.
• Symptomatic GERD: 20 mg once daily for 4 weeks. An additional 4 weeks may be considered if symptoms do not fully resolve. Once symptoms are controlled, an on-demand regimen of 20 mg once daily when needed can be used.
• Risk Reduction of NSAID-associated Gastric Ulcer: 20 mg once daily for 4-8 weeks.
• H. pylori Eradication (Triple Therapy): Esomeprazole 20 mg twice daily for 7-10 days, along with amoxicillin and clarithromycin.
• Zollinger-Ellison Syndrome: Starting dose is 40 mg twice daily, adjusted individually as needed.
• Duodenal Ulcer: 20 mg once daily for 2-4 weeks.
• Gastric Ulcer: 20-40 mg once daily for 4-8 weeks.

For adolescents (12-17 years), the dosage for GERD is similar to adults.

Nexum MUPS tablets should be swallowed whole with a glass of water and can be taken with or without food. For patients who have difficulty swallowing, the tablet can be dispersed in half a glass of non-carbonated water; stir until disintegrated and drink immediately or within 30 minutes. The pellets should not be chewed or crushed.

Side Effects:

Common side effects may include:

• Headache
• Diarrhea
• Nausea
• Flatulence
• Abdominal pain
• Constipation
• Dry mouth

Less common but potentially serious side effects can occur. If you experience any unusual symptoms, especially severe stomach pain, watery diarrhea, fever, changes in heart rate, muscle spasms, seizures, or signs of allergic reaction, seek immediate medical attention. Long-term use of PPIs like esomeprazole may be associated with an increased risk of bone fracture, low magnesium levels, and vitamin B12 deficiency.

Precautions:

• Use with caution in patients with severe liver dysfunction and severe renal impairment.
• Consider potential drug interactions, especially with drugs metabolized by CYP2C19 (e.g., diazepam, warfarin) and drugs whose absorption is pH-dependent (e.g., ketoconazole, iron salts, digoxin).
• Long-term use may slightly increase the risk of hip, wrist, and spine fractures.
• Inform your doctor if you develop any signs or symptoms suggestive of lupus.
• Not recommended for children under 12 years of age.
• Use during pregnancy should be with caution and only if clearly needed (Pregnancy Category C). It is not known if esomeprazole is excreted in breast milk.
