Napa Paediatric Drops contain Paracetamol 80 mg/ml. It is manufactured by Beximco Pharmaceuticals Ltd. in Bangladesh. 

Indications:

Napa Paediatric Drops are primarily used as an analgesic (pain reliever) and antipyretic (fever reducer) for infants and young children. It is indicated for: 

• Fever: Including post-vaccination fever. 
• Mild to moderate pain associated with conditions such as: 
  • Common cold and influenza 
  • Headache 
  • Toothache (e.g., teething pain) 
  • Earache 
  • Sore throat 
  • Body ache and muscle pain (myalgia) 
  • Post-operative pain 

Pharmacology:

Paracetamol (Acetaminophen) is believed to work mainly in the Central Nervous System (CNS) by increasing the pain threshold. It achieves this by inhibiting the synthesis of prostaglandins, which are chemicals involved in pain and inflammation. Its fever-reducing effect is primarily due to its action on the hypothalamic heat-regulating center in the brain, leading to peripheral vasodilation and sweating, which helps to dissipate heat. 

Dosage and Administration:

Napa Paediatric Drops are given orally. The exact dosage depends on the child's age and weight. Do not exceed 5 doses daily, and do not use for more than 3 days for fever or 5 days for pain unless directed by a doctor. 

• Up to 3 months: 0.5 ml (40 mg) 
• 4 to 11 months: 1.0 ml (80 mg) 
• 12 months to 2 years: 1.5 ml (120 mg) 

These doses should be given 3 to 4 times daily, or as directed by a physician. 

Side Effects:

Side effects of Paracetamol are usually mild and rare when taken at recommended doses. However, some reported side effects include: 

• Gastrointestinal disturbances (e.g., nausea, vomiting) 
• Allergic reactions (e.g., skin rashes, hives) 
• Blood disorders (rare, but can include thrombocytopenia (low platelets), leucopenia (low white blood cells), pancytopenia, neutropenia, and agranulocytosis) 
• Liver damage (with overdose or in patients with pre-existing liver conditions, especially with chronic excessive use or in combination with alcohol). 

Contraindications:

Napa Paediatric Drops are contraindicated in: 

• Known hypersensitivity to Paracetamol or any component of the formulation. 
• Patients with severe hepatic (liver) function impairment. 

Precautions and Warnings:

• Overdose: Taking more than the recommended dose can cause serious liver damage, which can be fatal. Keep out of reach of children. Seek immediate medical attention if an overdose is suspected, even if the child seems well. 
• Liver/Kidney Disease: Use with caution in children with pre-existing liver or kidney diseases. The dose may need to be adjusted by a physician. 
• Alcohol: Avoid alcohol consumption (for adults, if applicable, when taking other forms of Paracetamol) as it may increase the risk of liver damage. 
• Other Paracetamol Products: Do not give any other medications containing Paracetamol concurrently to avoid accidental overdose. Check ingredients of all cough and cold remedies. 

Pregnancy and Lactation:

• Napa Paediatric Drops are for pediatric use. However, for general information on Paracetamol: 
  • Pregnancy: Paracetamol is generally considered safe for use during pregnancy at recommended doses, and studies have shown low or no risk to the developing baby. Consult a physician. 
  • Lactation: Paracetamol is excreted in breast milk in small, clinically insignificant amounts. It is generally considered safe for use during breastfeeding, but consultation with a physician is advised. 

Drug Interactions:

• Warfarin: Regular prolonged use of Paracetamol may enhance the anticoagulant effect of warfarin and other coumarins, increasing the risk of bleeding. 
• Other Hepatotoxic Drugs: Concomitant use with other drugs that can damage the liver should be done with caution. 
• Certain Anticonvulsants (e.g., Phenytoin, Carbamazepine, Barbiturates) and Rifampicin: These drugs can increase the risk of liver toxicity from Paracetamol, especially with prolonged use. 

Storage Conditions:

Store below 30°C in a dry place, protected from light. Keep out of reach of children.
