Mycofree (Terbinafine): Advanced Allylamine Antifungal Therapy

Mycofree contains Terbinafine, a potent allylamine antifungal. It works by inhibiting the squalene epoxidase enzyme, disrupting fungal cell membrane synthesis. This causes a lethal accumulation of squalene inside the fungal cells, killing the pathogen.

📋 Formulations & Indications

  • Tablets: Treats fungal nail infections (onychomycosis) of fingernails or toenails caused by dermatophytes.

  • Granules: Indicated specifically for scalp ringworm (Tinea Capitis).

  • Cream & 1% Spray: Treats skin fungal infections including athlete's foot (Tinea Pedis), jock itch (Tinea Cruris), ringworm (Tinea Corporis), cutaneous candidiasis, and pityriasis versicolor.

🥄 Dosage & Administration Guidelines

  • Tablets (Adults): 250 mg once daily. Take for 6 weeks for fingernail infections and 12 weeks for toenail infections. Note: Healthy nail regrowth takes several months after finishing treatment.

  • Granules (6-Week Course based on weight):

    • Under 25 kg: 125 mg/day.

    • 25–35 kg: 187.5 mg/day.

    • Over 35 kg: 250 mg/day.

  • Cream & Spray (Topical): Clean and dry the area first. Apply a thin layer 1–2 times daily.

    • Treatment windows: 1 week for Tinea pedis/cruris/corporis; 2 weeks for candidiasis and pityriasis versicolor. Do not stop early to prevent recurrence.

🛑 Critical Warnings & Liver Safety

⚠️ Hepatotoxicity Risk: Rare cases of severe liver failure (sometimes resulting in transplantation or death) have occurred with oral Mycofree tablets. Pre-treatment blood tests (ALT and AST liver enzymes) are strictly advised for all patients. Discontinue oral treatment immediately if signs of liver injury appear.

  • Serious Skin Reactions: Stop treatment immediately if a progressive skin rash (like Stevens-Johnson Syndrome) develops.

  • Drug Interactions: Terbinafine inhibits the liver enzyme CYP2D6. Carefully monitor or reduce doses of co-administered tricyclic antidepressants, SSRIs, beta-blockers, and MAOIs.

🤰 Pregnancy & Lactation

  • Oral therapy should be postponed until after pregnancy. Topical use is only recommended if benefits outweigh potential risks. Terbinafine is excreted in breast milk; nursing mothers must not use it.

  • Store below 30°C in a cool, dry place. Protect completely from light and keep out of reach of children.
