Isentin M 500

Presentation: Each tablet contains linagliptin 2.5 mg and metformin hydrochloride 500 mg. 

Indications: Isentin M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both linagliptin and metformin is appropriate.  

Dosage and Administration: The dosage should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommended daily dose of linagliptin (5 mg) and metformin (2000 mg). It is usually administered orally twice daily with meals to reduce the gastrointestinal side effects associated with metformin. The recommended starting dose in patients not currently treated with metformin is one tablet of Isentin M 2.5/500 mg twice daily. In patients already treated with metformin, the starting dose should provide the same daily dose of metformin, with the addition of linagliptin 2.5 mg twice daily.  

Side Effects: Common side effects may include nasopharyngitis and diarrhea. Hypoglycemia may occur when Isentin M is used in combination with insulin secretagogues (e.g., sulfonylureas) or insulin. Lactic acidosis is a rare but serious potential side effect associated with metformin. Pancreatitis has been reported in some patients receiving linagliptin.  

Precautions: Renal function should be assessed before initiating Isentin M and periodically thereafter. Use is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min). Caution is advised in patients with a history of pancreatitis. Hepatic impairment, heart failure, and excessive alcohol intake are also important considerations.

Storage: Store below 30°C, protected from moisture. Keep out of reach of children.
