Hexirest 2 Tablet

Presentation: Each tablet contains Trihexyphenidyl Hydrochloride BP 2 mg.

Indications: Hexirest is indicated for the symptomatic treatment of:

• Parkinson's disease (idiopathic, postencephalitic, arteriosclerotic)
• Drug-induced extrapyramidal disorders

Dosage and Administration: The dosage is highly individualized and depends on the severity of symptoms and the patient's response. It is usually administered orally in divided doses, typically three or four times a day, preferably after meals.

• Parkinson's Disease: The initial dose is usually 1 mg on the first day, gradually increased by 2 mg increments at intervals of 3 to 5 days until a total daily dose of 6 to 10 mg is reached. Some patients, particularly those in the postencephalitic group, may require a total daily dose of 12 to 15 mg.
• Drug-Induced Extrapyramidal Disorders: The usual daily dose range is 5 to 15 mg per day.

Elderly patients and those with marked sensitivity to central nervous system drugs should start with lower doses.

Side Effects: Common side effects are due to its anticholinergic properties and may include:

• Dry mouth
• Blurred vision
• Constipation
• Urinary retention
• Tachycardia (rapid heart rate)
• Dizziness
• Drowsiness

Less common side effects may include nausea, vomiting, nervousness, and skin rash. Confusion or disorientation is more common in the elderly.

Precautions:

• Use with caution in patients with glaucoma (especially narrow-angle glaucoma), prostatic hypertrophy, urinary retention, gastrointestinal obstructive diseases, and cardiovascular disorders.
• May impair mental and/or physical abilities required for the performance of hazardous tasks, such as operating machinery or driving a motor vehicle. 
• Avoid abrupt discontinuation of the medication. Dosage should be gradually reduced under medical supervision.
• Use with caution in elderly patients who may be more susceptible to its side effects, particularly confusion and hallucinations.
• The safety of Hexirest during pregnancy has not been established, and it should be used only if the potential benefit justifies the potential risk to the fetus. It is notknown whether this drug is excreted in human milk.  

Contraindications: Hexirest is contraindicated in patients with:

• Hypersensitivity to trihexyphenidyl hydrochloride or any of the excipients.
• Narrow-angle glaucoma.
• Intestinal obstruction.
• Urinary retention.
• Severe tachycardia.

Drug Interactions: Hexirest may interact with other medications, including:

• Other anticholinergic drugs (may increase anticholinergic effects)
• Levodopa (may enhance dyskinesia)
• Certain antihistamines, phenothiazines, and tricyclic antidepressants (may increase anticholinergic effects)
