Hexinor Syrup - (2mg/5ml)

Presentation: Hexinor Syrup contains Trihexyphenidyl Hydrochloride at a concentration of 2mg per 5ml.

Indications: Hexinor Syrup is indicated for the treatment of:

  • As an adjunct in the treatment of all forms of Parkinsonism (postencephalitic, arteriosclerotic, and idiopathic).  
  • Control of extrapyramidal disorders caused by central nervous system drugssuch as dibenzoxazepines, phenothiazines, thioxanthenes, and butyrophenones.  

Dosage and Administration: The dosage should be individualized by a physician. The initial dose should be low and gradually increased, especially in patients over 60 years of age. Whether Hexinor is best given before or after meals should be determined by how the patient reacts.   

  • Idiopathic Parkinsonism: May start with 1 mg of Trihexyphenidyl on the first day. The dose may then be increased by 2 mg increments at intervals of three to five days, up to a total daily dose of 6 to 10 mg. The total daily intake is best tolerated if divided into 3 doses and taken at mealtimes. High doses (> 10 mg daily) may be divided into 4 parts, with 3 doses administered at meal times and the fourth at bedtime.  
  • Drug-Induced Parkinsonism: The size and frequency of the dose of Trihexyphenidyl should be determined empirically. The total daily dosage usually ranges between 5 and 15 mg, although, in some cases, these reactions have been satisfactorily controlled on as little as 1 mg daily.  

Side Effects:Common side effects may include dryness of the mouth, blurring of vision, dizziness, mild nausea, or nervousness.Patients with arteriosclerosis or a history of idiosyncrasy to other drugs may experience reactions such as mental confusion, agitation, disturbed behavior, nausea, and vomiting.Potential side effects also include constipation, tiredness, urinary hesitancy or retention, dilated pupils, increased intraocular pressure, vomiting, and headache.  

Precautions:

  • Use with caution in patients with arteriosclerosis or a history of drug idiosyncrasy, cardiovascular disease, glaucoma, gastrointestinal obstruction, prostatic hyperplasia, and/or urinary stricture.
  • Not intended for the treatment of tardive dyskinesia.  
  • Avoid abrupt withdrawal.  
  • Use with caution in hepatic and renal impairment and in elderly patients.
  • May impair mental or physical abilities, such as operating machinery or driving.
  • Perform gonioscopic examination prior to initiation of therapy and monitor intraocular pressure at regular intervals during prolonged treatment.

Contraindications: Hexinor is contraindicated in patients with hypersensitivity to Trihexyphenidyl HCl or any of the syrup ingredients and in patients with closed-angle glaucoma.

Drug Interactions: Increased antimuscarinic side effects may occur with phenothiazines, clozapine, antihistamines, disopyramide, nefopam, and amantadine. A synergistic effect may occur with concomitant use of tricyclic antidepressants (TCAs). Concurrent administration with MAOIs may cause dry mouth, blurred vision, urinary hesitancy or retention, and constipation. Trihexyphenidyl may antagonize the effect of metoclopramide and domperidone on gastrointestinal function and reduce the absorption of levodopa. It may also antagonize the effect of parasympathomimetics. Cannabinoids, barbiturates, sedatives, and alcohol may have additive effects.

Storage: Store as directed on the product packaging, usually at room temperature, away from moisture and heat. Keep out of reach of children.
