Glipita M 500
Presentation: Each tablet of Glipita M 500 typically contains 50 mg of sitagliptin and 500 mg of metformin hydrochloride. The tablets are film-coated and are available in blister packs. The appearance of the tablets (shape, color) may vary depending on the manufacturer.
Indications: Glipita M is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus when treatment with both sitagliptin and metformin is appropriate. It is used in patients whose blood sugar is not controlled well enough with metformin alone or who are already taking sitagliptin and metformin as separate tablets.
Dosage and Administration: The dosage of Glipita M should be individualized based on the patient's current regimen, effectiveness, and tolerability, not exceeding the maximum recommended daily dose of 100 mg of sitagliptin and 2000 mg of metformin hydrochloride. It is usually taken twice daily with meals to reduce the gastrointestinal side effects associated with metformin.
Side Effects: Common side effects may include upper respiratory tract infection, nasopharyngitis, and headache. Due to the metformin component, gastrointestinal side effects such as nausea, vomiting, diarrhea, and abdominal discomfort may also occur, particularly at the start of treatment. Hypoglycemia may occur when Glipita M is used in combination with insulin or sulfonylureas. Pancreatitis has been reported with sitagliptin.
Precautions: Glipita M is contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73 m²), acute metabolic acidosis, including diabetic ketoacidosis, and known hypersensitivity to sitagliptin, metformin, or any of the excipients. It is not recommended for use in patients with type 1 diabetes mellitus. Assess renal function before initiation and monitor periodically. Avoid excessive alcohol intake. Metformin should be temporarily discontinued in patients undergoing radiologic studies with intravascular administration of iodinated contrast materials or any surgical procedures necessitating restricted intake of food and fluids. Caution should be exercised in patients with a history of pancreatitis.
