Fosfogen (Fosfomycin Trometamol)

Indications
Indicated only for treating uncomplicated acute cystitis (UTIs) in women aged 18 and older caused by susceptible strains of E. coli and E. faecalis.
Dosage & Administration
- Dose: Single sachet for adult women (18+). Not for use in children.
- How to Take: Dissolve the entire sachet contents into 100 mL of purified water, stir well, and drink immediately. May be taken with or without food.
- Important: Do not take more than one single dose for an infection episode; daily doses increase side effects without improving results.
Mechanism of Action
Inactivates the enzyme enolpyruvyl transferase, blocking bacterial cell wall synthesis.
Contraindications & Warnings
- Avoid if: You have severe renal insufficiency, are on hemodialysis, or have a known hypersensitivity to the drug.
- CDAD: Clostridium difficile-associated diarrhea can occur with antibiotic use.
Interactions
- Metoclopramide (or other drugs that increase gastrointestinal motility) lowers Fosfogen’s serum concentration and urinary excretion.
Side Effects
- Common (>1%): Diarrhea (10.4%), headache (10.3%), vaginitis (7.6%), nausea (5.2%), rhinitis, back pain, dizziness, and abdominal pain.
- Less Common (<1%): Constipation, dry mouth, fever, vomiting, insomnia, and rash.
Pregnancy & Lactation
- Pregnancy Category B: Use only if benefits outweigh risks.
- Lactation: Decide whether to discontinue nursing or stop the drug based on maternal importance.
Storage
Store in a dry place below 25°C, protected from light. Keep away from children. Always take as directed by a physician.
