Denvar 400 is an orally active, third-generation cephalosporin antibiotic containing Cefixime. It's effective against a broad spectrum of Gram-positive and Gram-negative bacteria by interfering with their cell wall synthesis, and it's highly stable against beta-lactamase enzymes.

Uses: It's indicated for acute infections caused by susceptible microorganisms, including:

• Upper Respiratory Tract Infections (URTI): Such as otitis media and other URTI where resistance to common antibiotics is suspected or treatment failure carries significant risk.
• Lower Respiratory Tract Infections (LRTI): Like bronchitis.
• Urinary Tract Infections (UTI): Including cystitis, cystourethritis, and pyelonephritis.

Dosage & Administration:

• Typically taken for 7-14 days.
• Adults and children over 10 years: The recommended adult dosage is 200-400 mg daily, given as a single dose or two divided doses. (This means a Denvar 400 tablet might be used as a single daily dose or a 200mg tablet taken twice daily, depending on the doctor's prescription and the specific product formulation).
• Elderly: Same as adults, but assess renal function.
• Children: 8 mg/kg/day, single or two divided doses. For typhoid, 5 mg/kg body weight twice daily for 10-14 days. Children weighing >50 kg or >10 years get adult dose. Safety in children <6 months not established.
• Renal Impairment: For creatinine clearance <20 ml/min, do not exceed 200 mg once daily.

Side Effects: Generally well tolerated. Common side effects include diarrhea and stool changes (more frequent with higher doses), nausea, abdominal pain, dyspepsia, vomiting, flatulence. Less common effects include headache, dizziness, rash, itching, hives, drug fever, and joint pain. Serious but rare effects like pseudomembranous colitis can occur.

Contraindications: Do not use if allergic to cephalosporin antibiotics.

Precautions & Warnings:

• Use with caution in patients with hypersensitivity to other drugs, especially penicillin-sensitive patients due to potential cross-allergenicity.
• Administer cautiously in patients with markedly impaired renal function.
• Be aware of the risk of Clostridium difficile-associated diarrhea.
• Pregnancy and Lactation: Use only if clearly needed, as adequate studies in pregnant women are lacking, and it's unknown if Cefixime is excreted in human milk.

Interactions: Increases in prothrombin times have been noted, so caution is needed when co-administering with anticoagulant therapy.
