Daomin XR 500 

Presentation: Daomin XR 500 is an extended-release tablet containing Metformin Hydrochloride BP 500 mg as the active pharmaceutical ingredient. It is presented as an oval, white to off-white tablet and is available in blister packs. The extended-release formulation allows for once-daily dosing.

Indications: Daomin XR 500 is indicated for the treatment of type 2 diabetes mellitus, particularly in overweight patients, when dietary management and exercise alone do not result in adequate glycemic control. In adults, it may be used as monotherapy or in combination with other oral antidiabetic agents or with insulin. In children from 10 years of age and adolescents, it may be used as monotherapy or in combination with insulin. Metformin is known to reduce both basal and postprandial plasma glucose levels.

Dosage and Administration: The dosage of Daomin XR 500 must be individualized based on both effectiveness and tolerance, while not exceeding the maximum recommended daily doses. It should be administered orally once daily with the evening meal. The tablet should be swallowed whole and not crushed, cut, or chewed.

• Adults: The usual starting dose of Metformin XR is 500 mg once daily with the evening meal. Dosage increases should be made in increments of 500 mg weekly, up to a maximum of 2000 mg once daily with the evening meal, or alternatively increased to 1000 mg twice daily taken with meals. Patients receiving immediate-release metformin may be switched to extended-release metformin at the same total daily dose, up to a maximum of 2000 mg once daily.
• Children (over 10 years and adolescents): Metformin extended-release tablets have not been studied in children. Immediate-release metformin is usually started at 500 mg twice daily with meals, with gradual increases up to a maximum of 2000 mg per day in divided doses.

Renal function should be assessed before initiating treatment and at least annually thereafter. Daomin XR 500 is contraindicated in patients with an eGFR below 30 mL/min/1.73 m². Caution should be exercised if eGFR falls below 45 mL/min/1.73 m².

Side Effects: Common side effects associated with Daomin XR 500 may include gastrointestinal disturbances such as nausea, vomiting, diarrhea, abdominal pain, and loss of appetite, particularly at the initiation of therapy. These effects are often transient. A metallic taste may also occur. Long-term use of metformin may lead to vitamin B12 deficiency. Lactic acidosis is a rare but serious potential side effect.

Precautions: Daomin XR 500 is contraindicated in patients with hypersensitivity to metformin, severe renal failure (eGFR < 30 mL/min/1.73 m²), any type of acute metabolic acidosis (such as lactic acidosis, diabetic ketoacidosis), and acute conditions with the potential to alter renal function (e.g., dehydration, severe infection, shock). Caution should be exercised in patients with conditions that may cause tissue hypoxia (e.g., cardiac or respiratory failure, recent myocardial infarction), hepatic insufficiency, and acute alcohol intoxication. Alcohol may potentiate the effect of metformin on lactate metabolism.  

Storage: Store Daomin XR 500 tablets below 30°C, protected from light and moisture. Keep out of the reach of children. Do not use after the expiry date.
