Amit 25

Presentation:

• Each tablet contains 25 mg of Amitriptyline Hydrochloride BP.

Indications:

• Depressive illness, particularly where sedation is required.
• Nocturnal enuresis (bedwetting) in children (6-16 years).
• Prophylaxis of migraine.
• Tension headache.
• Chronic pain.

Dosage and Administration:

• Depression:
  • Adults: Initially 50-75 mg daily in divided doses or as a single dose at bedtime, gradually increased as necessary to a maximum of 150-200 mg daily. Usual maintenance dose is 50-100 mg daily.
  • Elderly and Adolescents: Initially 25-50 mg daily in divided doses or as a single dose at bedtime. Dose can be increased gradually as necessary.
• Nocturnal Enuresis:
  • Children (7-10 years): 10-20 mg at bedtime.
  • Children (11-16 years): 25-50 mg at bedtime, for up to 3 months and gradually withdrawn. A full physical examination should be conducted before further treatment.
• Prophylaxis of Migraine: 100 mg daily.
• Tension Headache: 10-25 mg three times daily.
• Amit 25 tablets should be swallowed whole with water.

Pharmacology:

• Amitriptyline Hydrochloride is a tricyclic antidepressant with sedative effects. It inhibits the reuptake of norepinephrine and serotonin in the brain, thereby increasing their concentration at the synaptic clefts. This action is thought to be the basis of its antidepressant activity. It also has anticholinergic properties.

Side Effects:

• Anticholinergic: Dry mouth, blurred vision, constipation, urinary retention, increased sweating.
• Cardiovascular: Hypotension, postural hypotension, tachycardia, palpitations.
• Central Nervous System: Drowsiness, dizziness, confusion, disturbed concentration, disorientation.
• Gastrointestinal: Nausea, vomiting, epigastric distress, constipation, diarrhoea, changes in appetite and weight.
• Other: Weakness, fatigue, skin rash, increased intraocular pressure.

Precautions:

• Use with caution in patients with a history of epilepsy, glaucoma, urinary retention, cardiac disease (particularly arrhythmias), diabetes, hepatic impairment, thyroid disease, and psychoses (may aggravate mania).
• May cause drowsiness. Patients should exercise caution while driving or operating machinery.
• Avoid alcohol consumption as it can enhance the sedative effects.
• Use with caution in elderly patients who may be more susceptible to side effects.
• Not generally recommended during pregnancy and breastfeeding; consult your doctor.

Contraindications:

• Recent myocardial infarction.
• Arrhythmias, particularly heart block of any degree.
• Mania.
• Severe liver disease.
• Concomitant use with monoamine oxidase inhibitors (MAOIs) or within 14 days of stopping MAOI treatment.

Storage:

• Store in a cool and dry place, below 30°C.
• Protect from light and moisture.
• Keep out of the reach of children.

Therapeutic Class:

• Tricyclic Anti-depressant.
