MTP Kit

Presentation: An MTP Kit typically contains two types of tablets: one tablet of Mifepristone and four tablets of Misoprostol. The Mifepristone tablet usually contains 200 mg of Mifepristone, and each Misoprostol tablet usually contains 200 micrograms (mcg) of Misoprostol. The kit is packaged together for the medical termination of pregnancy. Various manufacturers in Bangladesh produce MTP Kits under different brand names.

Indications: MTP Kit is indicated for the medical termination of intrauterine pregnancy up to 63 days (9 weeks) of gestation, calculated from the first day of the last menstrual period.

Dosage and Administration: The MTP Kit should be used strictly under the supervision of a qualified medical professional and according to their instructions. The typical regimen involves:

• Day 1: One tablet of Mifepristone (200 mg) is taken orally in a single dose.
• Day 2 or 3 (24-48 hours after Mifepristone): Four tablets of Misoprostol (200 mcg each, total 800 mcg) are administered buccally (placed between the cheek and gum) or sublingually (under the tongue) and allowed to dissolve for about 30 minutes. The remaining fragments can be swallowed with water. In some cases, vaginal administration of Misoprostol may be recommended by the doctor.

Patients should be advised about the expected bleeding and cramping, which may be heavy. Pain relief medication may be needed. A follow-up visit to the doctor is essential to confirm the completion of the abortion.

Side Effects: Common side effects associated with the use of MTP Kit include:

• Abdominal pain and cramping
• Heavy vaginal bleeding, which may include blood clots
• Nausea
• Vomiting
• Diarrhoea
• Dizziness
• Fatigue
• Headache
• Fever or chills

Severe side effects are rare but can occur, including very heavy or prolonged bleeding, infection (fever, foul-smelling vaginal discharge), incomplete abortion, and allergic reactions. Any severe symptoms should be reported to a healthcare professional immediately.

Precautions: MTP Kit is contraindicated in certain conditions, including:

• Pregnancy beyond 63 days of gestation
• Confirmed or suspected ectopic pregnancy
• Known allergy to Mifepristone or Misoprostol
• Severe asthma uncontrolled by therapy
• Inherited porphyria
• Chronic adrenal failure
• Severe liver, kidney, or heart disease
• Uncontrolled hypertension
• Haemorrhagic disorders or current anticoagulant therapy
• Presence of an intrauterine device (IUD) (which must be removed before using MTP Kit)

Caution should be exercised in women with anaemia or bleeding disorders. The use of MTP Kit requires medical supervision to ensure proper use and to manage any potential complications.

Use in Pregnancy and Lactation: MTP Kit is used to terminate pregnancy and is therefore contraindicated for the continuation of pregnancy. It is not recommended for use during lactation as it is unknown if Mifepristone or Misoprostol are excreted in breast milk and what effects they may have on a nursing infant. Breastfeeding women should consult their doctor for advice.

Storage: The MTP Kit should be stored in a cool and dry place, protected from light. Keep out of reach of children.
